Patient Reminders
- Providers are in different lobbies every day. Please check the directory board on first floor by the elevator to seee where you need to check in for your provider.
- Please be respectful of other patients and practice social distancing.
- Everyone who enters the building must wear a mask over mouth and nose.
- Only patients are allowed in the building unless medically necessary with only one adult.
- Please do not use your cell phone in witing rooms or exam rooms.
- Please do not arrive more than 10 minutes prior to appointment time.
- Copays are due when checking in.
- Please have your insurance cards and photo identification available when checking in.
